Spinal surgery succeeds in relieving pain for approximately 50-80% of carefully selected patients. However, studies indicate that 10-40% of individuals who undergo lumbar spine surgery continue to experience significant pain afterward. The reasons are multifactorial and often involve a combination of structural, neurological, and physiological factors.
Epidural fibrosis — the formation of scar tissue around spinal nerve roots — occurs in nearly all post-surgical patients to some degree. When excessive scar tissue develops, it can compress or tether nerves, creating new pain generators that were not present before the operation. This scarring process typically evolves over 6 to 12 weeks following surgery.
Adjacent segment disease is another common contributor, where spinal levels above or below the surgical site undergo accelerated degeneration due to altered biomechanics. The fused or altered segment transfers increased stress to neighboring vertebrae, potentially causing new disc herniations, facet joint arthropathy, or spinal stenosis at those levels.
